If you are interested in receiving a 1st dose vaccination, 2nd dose vaccination, or booster shot next week Thursday 11/24/21. Walk-ins will also be accepted on the day of the clinic between 3:00PM - 4:15PM.
Please bring the following:
Picture ID
Completed forms
Insurance card (if applicable, insurance is not required)
Vaccination card (if returning for a booster)
Below are electronic copies of forms needed for the clinic please fill them out and bring to the clinic.
Booster Attestation Form (only for those receiving a booster)
Student Consent Form (only for those under 18 years of age)
