This is great timing — my kid's school just sent home something similar and I spent way too long squinting at the survey questions trying to figure out what "adequate" really means on a scale of 1-5. Speaking of quality assessments, I actually found myself thinking about how we judge things like images too — there's a whole tool for making low quality images on purpose these days, which feels like the opposite problem.
